The current form is for clinic owners and managers discussing a business pilot. Do not submit patient names, symptoms, diagnoses, identifiers or medical documents. The reception simulation collects no patient details and makes no real appointments.

Information and purpose

The form collects your practice name, contact name, business email, optional website, country and reply language. It also records when you submitted, the notice version and optional campaign labels from the link. SZY & Associates uses these details to review and respond to your business inquiry. Submission is not a purchase or a newsletter subscription.

Access and retention

Inquiries are stored in the site's database and shown only in the restricted owner inbox. They are not forwarded to an AI model, messaging service or calendar in this version. Entries older than 90 days are removed on the next inbox view or form submission. The owner can also delete an entry immediately.

The site uses platform hosting and sign-in services. No marketing trackers have been added to the application. This summary does not replace the hosting provider's notices.
